Helping suddenly bereaved people
Thank you for making a referral, on behalf of a bereaved person, to Sudden’s service, for people bereaved in sudden or too-soon ways. This form is confidential.

Please note we are not a mental health nor counselling service. We offer practical and emotional trauma-informed bereavement support within the first ten weeks of an unexpected bereavement. For all referrals we ask for details relating to a bereaved person’s health, to enable us to offer informed support and work with agencies such as GPs, or mental health or social care professionals.
After sending this form, one of our Sudden bereavement case workers will acknowledge receipt, and, if necessary, discuss it with you before we make contact with the bereaved person directly.
The case worker will then contact the bereaved person directly to assess their needs and make a care plan. We will then provide emotional and practical support for up to ten weeks from the date of their bereavement, including accessing support from other agencies as needed, during our care and when someone exits our care.
